a friend of mine had this problem in an other game with a much better GPU than mine. Actually the GPU was not totally broken , but some units were though. So the GPU was running fine in idle, but it was completely lagging in load. But in your case it's only tf2 so its not the problem

I seriously recommend trying defrag your CFGs as most people don't do it and can help performance not going to say increase FPS but can help make more stable http://www.youtube.com/watch?v=ccmoToUgEpo whole guide is in the video including download links. This works similar to what tapley suggested of deleting your whole game and starting again.
I do this every update tf2 gets. I've closed all processes in the background that is not needed. I don't even want 120, I just want a consent 60fps. to stay like that even during mid fights. My PC is clean, I clean it every fortnights, no massive dust shit none of that. Processes in background are not that many, they only consist of antivirus, drivers, windows important shit. 120fps with q6000 q8000 or q9000 2.5ghz is impossible ur kinda making a problem where there isnt any, tf2 needs good core speeds and u dont have any... u simply dont have good enough cpu, no cfg or setting will help

Run tf2 in windowed modus, go to windows task manager and put cpu usage of hl2exe to high. Did the trick for me :p
