Afaik it began at The Reality lan (that's where I heard it the first time). TCM and TmG were sitting at the same table across each other. Byte went down at one point and someone (I think it was Skre) yelled "Byte down, puuuuuuuuuuuuuuuuuuuuuuuush!"
